  • Decree of Ministry of Justice and Human Rights on Deed of Amendment / Establishment
  • Director's ID (KTP) or passport
  • Director's Tax ID (NPWP)
  • Company Tax ID (NPWP)
  • NIB/SIUP/TDP
  • Other business licenses according to business activities
Foundation (Education, Health Services, Religious Activities, Social Fund)
  • Latest Company Deed
  • Decree of Ministry of Justice and Human Rights on Deed of Amendment/Establishment

  • Chairman's Tax ID (NPWP)
